Certificates
By law, the University can only issue revenue stamp certificates, which can only be used in private relationships, while in relations with public administration bodies and public service providers, certificates and affidavits are always replaced by the declarations referred to in Articles 46 and 47 of Presidential Decree no. 445/2000 (Consolidated Law on Legislative and Regulatory Provisions on Administrative Documentation).

It is possible to request certificates of attendance, enrolment with or without the amount of the scholarship, and degree in Italian or English by sending a request by e-mail to the address of the Doctorate Office: dottorati.ricerca-mi@unicatt.it

- Enrolment application
Enrolment application to be completed only in case of technical problems with the online enrolment process.
- Research program
Upon enrolment, one must be authorised by the Coordinator to carry out a research programme during the PhD programme. The form, duly signed, must be delivered to the PhD Office.
- Model 2 Prevention and Protection Service
Form 2 Prevention and Protection Service: the PhD student, immediately after enrolment, is required to fill in the following documentation: Form 2 (mandatory) Tab A (if necessary) Form B (if necessary). The documentation must then be signed by the Coordinator (or by the responsible teacher delegated to do so) of the PhD programme in which the PhD student is enrolled and sent by email to: dottorati.ricerca-rm@unicatt.it Research Doctorate Office and ser.sicurezza-rm@unicatt.it Prevention and Protection Service. The information contained in the forms is necessary and indispensable to proceed with a correct assessment of occupational risks and therefore with a possible medical examination for health surveillance, preparatory to the start of the PhD activity. This form, relating to the activity carried out by the PhD student during his or her attendance, must be filled in again only in the event of changes in the performance of the activities during the course of the doctorate. - Budget for research activities - 10% VALID FROM THE 38TH CYCLE.
"Starting from the first year of the 38th cycle (A.Y. 2022/2023), each PhD student is ensured, in addition to the scholarship and within the financial resources existing in the budget of the entities accredited under current legislation, a budget for research activities in Italy and abroad adequate with respect to the type of course and in any case of an amount of not less than 10% of the amount of the scholarship itself. If the doctoral student is not positively evaluated for the purpose of renewing the scholarship, or renounces it, the unused amount remains at the disposal of the institution, for the same purposes"
Form to be filled in, signed by the Coordinator of the PhD course and forwarded to the PhD Office at least one month before departure.
The participation of doctoral students in the medical area, whether with or without a scholarship, graduates in medicine and surgery or in dentistry and dental prosthetics, in clinical-assistance activities is allowed subject to the authorization of the Academic Board.
The interested PhD student must submit the request to his/her coordinator by filling in the appropriate form.

Doctoral students who have been awarded a scholarship must submit the Tax Registry Form (MAF) at the time of enrolment and, subsequently, in the event of any change in tax and bank data.
Declaration in lieu of certifications
As of 1 January 2012, with reference to the provisions of art. 15 of Law no. 183/2011, in relations with public administrations and managers of public services, certificates are replaced by declarations in lieu of certifications. Please note that self-certifications, in order to be valid, must always be signed in original by the interested party and presented together with a photocopy of a valid identity document.
To facilitate the PhD student or PhD, the following self-certification forms have been prepared:
The certificates, requested for private use, will be issued on letterhead and stamped duty, upon presentation of a specific stamped request.

Application for study and research abroad
Form to be filled in, signed by the Coordinator of the PhD course and forwarded to the PhD Office at least one month before departure.
Fill the form at least 1 month before journey and bring it to the PhD Office.
Budget for research activities - 10% - FOR CYCLES PRIOR TO 38
Ministerial Decree no. 45 of 8 February 2013 [art. 9 point 3 (scholarships)] Regulation on the accreditation of doctoral venues and courses and criteria for the establishment of doctoral courses by accredited bodies.
"Starting from the second year*, each PhD student is ensured, in addition to the scholarship and within the financial resources existing in the budget of the accredited subjects under current legislation, a budget for research activities in Italy and abroad adequate to the type of course and in any case of an amount of not less than 10% of the amount of the scholarship itself. If the doctoral student is not positively evaluated for the purpose of renewing the scholarship, or renounces it, the unused amount remains at the disposal of the institution, for the same purposes".

Application for clinical-care activities
The participation of doctoral students in the medical area, whether with or without a scholarship, graduates in medicine and surgery or in dentistry and dental prosthetics, in clinical-assistance activities is allowed subject to the authorization of the Academic Board.

Form for authorisation to submit a thesis in a foreign language
Doctoral students may write their doctoral thesis in a foreign language, subject to the authorization of the Academic Board.
The following form, therefore, before being delivered to the PhD Office, must be completed in all its parts, signed by the interested party and signed by the PhD Coordinator.
Admission form for the European PhD
By 30 September of the last year of the Doctoral Programme, an application can be submitted to the Doctoral Office to be admitted to the Doctor Europæus Label, having fulfilled the required conditions.
PhDs , after obtaining the degree, can request the parchment by filling in and delivering or sending the following form to the PhD Office.
Request form for visiting PhD student
PhD students from other universities (Italian and foreign) may request to carry out research exclusively at an Institute or Department of Università Cattolica del Sacro Cuore, subject to the authorisation of the Director, by filling in the attached form and submitting the documentation required therein.
Application for suspension of attendance
Form, with revenue stamp, to be completed and sent to the PhD Office for forwarding the PhD course to the Academic Board for the required authorization.
Application for withdrawal from studies
Form, with revenue stamp, to be filled in and sent to the PhD Office, together with the badge.
In case of withdrawal from the scholarship during the year, it is necessary to fill in the form and send it by email to the PhD secretariat and a copy to the PhD course coordinator.
The waiver is valid for the entire calendar year and is irrevocable.
In order to guarantee 100% of the scholarship to all doctoral students who have applied for maternity/paternity suspension, it is proposed to the Board of Directors that the University integrate, to the extent of 20%, the allowance paid by INPS for the 5 months of suspension of the scholarship, by means of a specific request to be made by the interested parties to the competent administrative offices, using the appropriate form.
